Please review the clinical placement definitions below and click on the title that describes your Oakwood clinical assignment. A page listing all of the orientation documents you need for your clinical placement will appear. You may review the orientation information from the website or print and review the paper version of the information. All information, including forms, can be printed and copied by you as needed for your clinical experience.
Please review the documents for your clinical placement and ensure you are knowledgeable of this orientation information. If you are faculty, it is your responsibility to ensure your students are also knowledgeable of this information. As faculty, please be sure to contact the manager of your assigned unit/department for your clinical placement as soon as possible in order to arrange a date and time for your unit/department orientation. If you are a student who will not have on site faculty here with you during clinicals, then it is your responsibility to determine who your Oakwood preceptor/mentor will be and contact that person for unit/department orientation.
You must turn in all required paperwork to your clinical site coordinator prior to the start of your clinical experience. Your clinical experience cannot begin until all required paperwork is received by Oakwood.

If an Oakwood nurse been particularly helpful to you, kind to you, supportive of your efforts to become a nurse and you would like to send a special thank you to them, please submit this form with your comments. Even if you don’t know his or her name, we’ll do our best to take your note to that nurse and we’ll make sure to inform his or her manager and peers. Thank you for taking time to recognize their positive efforts. It means a lot to these nurses and nurse managers to hear your positive feedback.
